Transvenous Lead Extraction of Cardiac Implantable Electronic Devices: Who, When, How and Where?
نویسندگان
چکیده
The use of cardiac implantable electronic devices (CIED) has greatly increased over the past 2 decades. In parallel, there has been a growing need for device and lead extraction. The subsequent development of tools, techniques, and standards for transvenous lead extraction (TLE) has improved both the safety and efficacy of these procedures. However, these remain complex interventions that require well trained personnel and a well-equipped facility with tools and strategies to deal with life-threatening complications, should they occur. In this condensed review, we discuss the topic of TLEs with an emphasis on the key elements including: a) qualifications of the primary operator; b) procedural indications and factors to account for in decision making, as well as the c) tools, and d) techniques for TLE: ‘‘The who, when, how and where?’’

Partial extravenous course of cardiac pacemaker leads. A major risk during device-assisted extraction
Introduction The intracardiac electronic device implantation rate has been constantly increasing for the last 20 years and is accompanied by an increasing lead extraction rate. Percutaneous extraction of transvenous leads of cardiovascular implantable electronic devices is mandatory in the case of pocket infection and/or system infection, resulting in valvular or lead endocarditis. Lead extract...
متن کاملTransvenous Lead Extraction via the Inferior Approach Using a Gooseneck Snare versus Simple Manual Traction
BACKGROUND AND OBJECTIVES The number of patients with cardiac implantable electronic devices needing lead extraction is increasing for various reasons, including infections, vascular obstruction, and lead failure. We report our experience with transvenous extraction of pacemaker and defibrillator leads via the inferior approach of using a gooseneck snare as a first-line therapy and compare extr...
متن کاملImplantation of a leadless cardiac pacemaker for recurrent pocket infections
Infections involving cardiovascular implantable electronic devices (CIEDs) remain an unfortunate indication for repeated device-related procedures, including extractions and reimplantations. Recurrent infections involving transvenous leads and superficial device pockets may eventually lead to epicardial lead implantation and generator placement in deeper tissue planes, both of which require a m...
متن کاملOutcomes of Transvenous Lead Extraction for Cardiovascular Implantable Electronic Device Infections in Patients With Prosthetic Heart Valves.
BACKGROUND Lead-related or valve-related endocarditis can complicate cardiovascular implantable electronic device (CIED) infection in patients with both CIED and prosthetic valves. The objective of this study was to determine the outcomes of transvenous lead extraction for CIED infection in patients with prosthetic valves. METHODS AND RESULTS We retrospectively screened 794 transvenous lead e...
متن کاملLeadless Pacemakers: practice and promise in congenital heart disease
The development of the implantable cardiac pacemaker nearly 60 years ago represented a paradigm shift in the control of potentially lethal heart rhythms and has revolutionized the lives of those at risk of sudden death from bradycardia. Subsequent technological developments have resulted in devices that offer additional benefits to patients, including defibrillation and resychronisation therapy...
